Person-Directed Care

Person-Directed Care is a model to improve nursing home culture using a Holistic Approach to Transformational Change (HATCh) philosophy. This model combines quality improvement methodologies with both the science and psychology of change. The focus of this model is to move away from an institutional model that revolves around the tasks, schedules and systems of delivery related to medical care and illness. The goal is creating an individualized care model that addresses the human spirit, needs, strengths and necessity of connectedness, relationships and growth at the heart of every person, all people - staff, residents, and families.

Relationships are the heart of the model. Before people care about what you know, they must know that you care.
